An alter-ego for Mistress Alexandra. She is trained as a doctor, and has worked as a nurse, so she is more than qualified, so the role is not a sham.
In this guise, Mistress Alexandra is demure and dressed in pure white (though you do have to shave to wear that dress and I don't mean the legs).
